Videos must be recorded within 14 days prior to the date of the test.

Step 1: Read all the instructions and familiarize yourself with the requirements.​

Step 2: Download and print the forms you will need to submit with your test application.  

• Performance Affidavit

• Name and Likeness Release and Consent Agreement

• Permission to Test​
   

Step 3: Record your video.​

• Fill out your affidavit and get it signed by the proctor, videographer and your coach.

• PLEASE read the Virtual Testing Video Requirements above. We will have to reject any video that is not recorded correctly. (This means you MUST record in landscape orientation.)

• Don't forget the NEW USFSA REQUIREMENT: All tests recorded for submission to a virtual test session are required to include a Virtual Test Placard. The placard must include the information below and must be held in front of the camera prior to the date (month and day) and time, which is also required. Information can be printed or handwritten on a piece of paper or on a whiteboard as long as the following information is included. (A sample placard can be downloaded from USFSA here.)

Step 4: Fill out and get the Name and Likeness Release and Permission to Test forms signed. 

Step 5: Scan or take a photo of your forms.

Step 6: Submit your application and signed paperwork to testchair@cortlandskating.com OR mail in a hard copy of your application and forms.​​​

• You will receive an email confirmation once you have submitted this form. If you do not receive an email confirmation, you are not registered.

Step 7: Submit your video(s).

• Submit your video electronically (use a google drive, dropbox folder, or email to testchair@cortlandskating.com). Please email us if you need us to supply a link to a google drive for uploading.
   

Step 8: Submit payment for your test(s). 

• Pay by check (Made payable to Cortland Figure Skating Club and sent c/o Jennifer King MacKenzie, 17 Braeside Drive, Homer, NY 13077) 

• Pay via Venmo (@cfsc13045 and include an additional $5 processing fee)

Payment, video, and ALL required forms must be received by MIDNIGHT on the date of the Virtual Test. Please note that failure to send in all forms/quality video may result in test not being judged. Questions? Email testchair@cortlandskating.com.
